Chronic reflux disease is a common condition in the general population that may impact on patient quality of life. To examine the evidence for the effect of surgical treatment for gastro-oesophageal reflux disease (GERD) on quality of life. We searched Medline, Embase and the Cochrane library from March 2009 to August 2012. This systematic review included randomised controlled trials that assessed patient reported outcomes (HRQL) following laparoscopic antireflux surgery, laparoscopic Nissen fundoplication (LNF), laparoscopic Toupet fundoplication (LTF) or laparoscopic Heller myotomy (LHM). We excluded trials in children. Two reviewers selected studies and assessed study quality using the Cochrane risk of bias tool. Data were extracted using a standardised form. The review included six studies of which two were at low risk of bias. In these two studies, laparoscopic antireflux surgery resulted in significantly better outcomes in several HRQL measures including the Gastroesophageal Reflux Disease Health Related Quality of Life Questionnaire (GREQLQ), Reflux Symptoms Questionnaire, Gastrointestinal Quality of Life Index and Gastroesophageal Reflux Disease Impact Scale (GERDIS). In two further studies of laparoscopic antireflux surgery, there was a borderline improvement in the GERDIS score (P = 0.09). In the remaining two studies there was no improvement in the study outcome measures. Laparoscopic antireflux surgery results in significant improvements in HRQL in selected patients with GERD.1.
